Neurofilaments (Nf) are an intermediate filament protein of the cytoskeleton, integral to the structure and function of axons (and hence, neurons). Nf are composed of four different subunits: Nf- light chain (NfL), Nf- medium chain (NfM), Nf- heavy chain (NfH), and peripherin . During neurodegeneration, which can occur in neurological disease, neurons break down. This process of neural breakdown releases NfL into the blood and CSF. It is through samples of the plasma, serum, or CSF that NfL levels can be detected in an individual. NfL in blood samples can be measured using multiple different analytical platforms including: ELISA, electrochemiluminescence immunoassay, or Simoa NfL assay . Elevated NfL levels in both the blood and CSF have been used as a biomarker for the presence and severity of a neurological disorder, with both blood and CSF NfL levels having similar diagnostic performance for several disorders. NfL levels also correlate with other biomarkers, such as neurofibrillary tangles, parahippocampal brain atrophy, and white matter lesions .
